Automatic detection of ischemia in ECG

This thesis discusses the utilization of the artificial neural networks (ANN) for detection of coronary artery disease (CAD) in frequency area. The first part of this thesis is orientated towards the theoretical knowledge. Describes the issue of ECG pathological changes. ECQ are converted to frequency area. Described statistical methods and methods for automatic detection of CAD and MI. Explained the issue of the perceptron and ANN. The second deals with use of Neural Network Toolbox MATLAB®. This part focuses on counting and finding suitable parameters and making connection of band. At the end of the thesis UNS is used to detect ischemic parameters and the results are discussed. Average values for the best settings are 100% accuracy.

Tato bakalářská práce se zabývá genetikou onemocnění koronárních tepen. Díky studiu rizikových genetických variací je možné identifikovat ohrožené pacienty a vyvíjet cílenou léčbu. V první kapitole se zmiňuje anatomie, histologie a patofyziologie koronárních tepen. Druhá kapitola je zaměřena na dosavadní znalosti dědičnosti onemocnění koronárních tepen. Ve třetí kapitole jsou zmíněny dva genomické projekty, které přispěly ke studiu variací u onemocnění koronárních tepen, a způsoby čtení dat z jejich databází. Těmito projekty jsou 1000 Genomes Project a CARDIoGRAM plus CD4 consortium. Na základě dat z 1000 Genomes databáze byl získán referenční panel vazebné nerovnováhy. Dále byla provedena asociační analýza na základě dat ze souhrnné statistiky metaanalýzy provedené CARDIoGRAM plus CD4 konsorciem.

The first part of this thesis is orientated towards the theoretical knowledge and describes the issue of ECG pathological changes, methods for automatic detection of CAD and MI and the issue of the perceptron and ANN. The second deals with use of Neural Network Toolbox MATLAB® version R2010a. In graphical user interface development environment (Guide) is created application that is used to compare the success of automatic detection of ischemia in ECG using ANN. It allows the user to set various parameter settings UNS and display ECG waveforms.
Lipidomic analysis of adipose tissue in patients with ischemic heart disease

The ever-increasing incidence of coronary artery disease (CAD) in Czech Republic and worldwide is one of the most important health problem today. Obesity and type 2 diabetes mellitus (DM2T) are risk factors for the development of CAD. Lipid metabolism is greatly affected by these diseases and adipose tissue, previously considered a mere energy store, also plays an important role in these changes. Today adipose tissue is discussed also as an endocrine organ, which effect whole body metabolism significantly. Lipidomic analysis is an effective tool for monitoring lipid metabolism. Lipidomics combines the advantages of determining a large number of analytes and a small amount of sample and its rapid preparation. In the first part of study, I optimized the HPLC-MS method of untargeted lipidomic analysis in adipose tissue. Then, this method was used to compare subcutaneous and epicardial adipose tissue. Finally, I evaluated the effect of CAD and DM2T on the lipid profiles of subcutaneous and epicardial adipose tissues. Optimization of the pre-analytical phase enabled the analysis of 206 lipid molecules (in total) in both types of adipose tissue. The knowledge of patients with coronary artery disease on the importance of nutrition and lifestyle.

Coronary artery disease is the most common cause of premature death, not only in our country but worldwide. The purpose of this thesis is to determine the habits of patients with this serious disease, the impact of education on their knowledge needed to change their current lifestyle and diet, success and possible barriers in putting these changes into practice. The easiest approach and conditions to the education in a physical activity area, lifestyle changes and nutrition have those patients with indicated spa treatment. For these reasons the research was carried out among patients treated in the spa Poděbrady through three consecutive surveys: in the beginning, at the end and three months after the end of the spa cure. The responses were carefully analysed. 45 patients with coronary artery disease and 22 patients after valvular disease surgery were involved in this study as a control group. With the further progress of the research the interest of patients and the response rate declined. Hazardous behaviour like smoking, lack of physical activity, increased salt intake, excessive energy intake of the patients with coronary artery disease was confirmed by this study. Analysis of selected genetic markers in patients after heart transplant

Heart transplantation is performed in patients with end-stage heart failure, in whom all other methods of treatment failed. The most common causes of end-stage heart failure are dilated cardiomyopathy and coronary artery disease. The destiny of these patients is highly variable. Prediction of long term survival in patients after heart transplantation is not satisfactory and up to now has not been found reliable marker. Most of the patients die after heart transplantation due to cardiovascular disease. This thesis is focused on molecular genetics and statistical analysis of four single nucleotide polymorphisms, namely rs17817449 (16q12.2, FTO gene), rs2943634 (2q36.3; intergenic region), rs6922269 (6q25.1; MTHFD1L gene), and rs10757274 (9p21.3; intergenic region). According to genome wide association studies are these SNPs assosiated with cardiovascular diseases. We genotyped DNA samples of 364 heart donors and 364 heart recipients. The results were statistically compared (using OR and Pearson's χ2 test) with the control group, which consisted of samples of individuals from the general population MONICA study. The influence od educational to maintenance of secondary prevention rules of myocardial infarction

This bachelor thesis deals with the effects of educational plans on the compliance with the principles of the secondary prevention of myocardial infarction. The theoretical background covers atherosclerosis, acute myocardial infarction, the secondary prevention of myocardial infarction and the educational process in nursing. Prior to elaborating the practical part of the thesis we had established two objectives. Objective 1: To find out whether the patients educated without an educational plan are as knowledgeable as the patients educated through an educational plan. Objective 2: To compare whether the patients educated without an educational plan comply with the principles of the secondary prevention of myocardial infarction in the same way as the patients with an educational plan. A qualitative form of research had been chosen for this thesis, semi-standardized interview in particular. The analysis of the data gained was subsequently carried out. Two objectives had been established in the thesis. Objective 1: To find out whether the patients educated without an educational plan are as knowledgeable as the patients educated through an educational plan. Through the analysis of the obtained data we found out that the patients educated without an educational plan are as knowledgeable as the patients educated through an educational plan. Objective 2: To compare whether the patients educated without an educational plan comply with the principles of the secondary prevention of myocardial infarction in the same way as the patients with an educational plan. Having compared the results we found out that the patients with an educational plan comply with the principles of the secondary prevention more consistently than the patients educated without an educational plan. The objectives of the thesis were met.
